This was a mistake from the very beginning putting my daughter in this facility for rehab. After about 3 weeks into her rehab they submitted a progress report to BC/BS that she was making no progress and that she wasn’t a “fit” for their facility. The Social Worker gave a negative report to another facility that we were trying to get her moved into. Based on social workers info this nursing home didn’t feel like they could accept her. Do you research folks and read all the negative comments that people are saying about this facility. I’ve read them and they are all right on the mark. Should have the license to operate pulled for such poor service and at a rate of $195 per day for a semi private room. I could write a short story on how rude and condescending their Social Worker, Business manger and Director of Nurses were but I’ll leave it at that. Beware folks not as it appears.

Don't send your loved ones here; the place is run like a prison and I'm sure it's not any different based on the care that my father-in-law received. I understand that during these times, healthcare facilities are understaffed and have a lot on their hands but there's a fine line between that and neglecting your responsibilities. My daughter recorded her experiences at this place and it is just cruel to let these people stay here. Once you're in this place, you'll have to jump through loops to get them out. We couldn't even get ahold of how my father-in-law was doing until he was dying. The nurse's station will often have no one attending it. If you're lucky enough to find one, you'll be ignored because they're too busy talking/arguing with each other. If they don't ignore you, they'll tell you to go back to the room you came from and wait for them to come to you (as if we didn't already do that and only came to look for them BECAUSE no one came by.) And this was treatment towards a VISITOR. There were other patient rooms with their call lights on for who knows how long. I can only imagine how little care they give to their residents. Based on the other (accurate and realistic) reviews of this place, I'm sure the person in charge of their online material will reply to this. I have evidence of this neglect along with footage of the nurses arguing for over 5 minutes and then abandoning their post. The pictures included here is just one of the many rooms that the residents stay in. My family is desperately trying to get a loved one out of here, please do not allow these people to care for your family. We've made a terrible mistake and hope that no one else will do so here.

Having never been through this, we were clueless and had only heard horror stories. After having a severe heart attack, uncontrolled diabetes, and Alzheimer's our Mom needed 21 days of rehab after 20 days in hospital. Legend Oaks was one of three facilities referred to us. My husband and I went there 1st and immediately knew this was the best place for Mom! The care was phenomenal, concerns and questions were addressed immediately and the followed up on to make sure all concerns were handled! They always were, and there were very few! BIG SHOUT OUT TO THERAPY TEAM! They are the best! We were able to have same therapist give home care and now our Mom is going 3x a week to Legend Oaks for out patient therapy! She has improved soooo much! A SPECIAL SHOUT OUT TO [name removed], [name removed], AND [name removed]! She would not be where she is without your awesome skills, compassion, and concern! My mother has been at Legend Oaks for a year, main issue is that she is supposed to have a shower 3 times a week. This rarely happens. She has gone over a week with no shower at all several times and is lucky to get one a week when I email or call complaining. She is always telling me that they are out of tea or out of sugar or out of eggs. They know she will not eat scrambled eggs and sausage and will bring her that instead of a fried egg and then tell her there are no eggs so she just doesn't get to eat. I have complained to management because she is supposed to get a menu for meals and never give her a menu or if I complain and she gets one or two of them they don't feed her what is on menu. She has had several uti's due to cleanliness (because of lack of showers), but I am the one that has to tell them to check her UTI. Currently I have been waiting for over a week for her to be tested and it has not been done yet, which causes her to sundown, because it is obvious to me she has one. I have email both directors of nursing and administrator with no response to anything at all since last Friday. This is another ongoing problem, no return calls or emails in regards to care of if do respond do not do what they say will do. Very understaffed and large turnover of all employees except the receptionists that are nice.

Great place to have your loved one. I was there visiting a friend. I was very impressed how friendly the staff is there. Your greeted with a smile. All the staff says hello in passing ask if you need anything. Beautiful place felt warm and inviting. I will recommend Legend Oaks Garland.

This facility started out great. My mom moved in in May. Soon the problems started with all of the staff. There's a great turnover in staff. They don't do their jobs and ignore the patients. In the dining room, food is served late and cold--what food there is. High carbs and very few choices. The staff stands around talking instead of assisting the residents with getting their meals, drinks, disorderly patients, etc. Mom was given approval by the doctor to bring her own meds there. She did and turned them over to her nurse who turned them over to the Nursing Administrator to lock up. Those meds are nowhere to be found, and our calls are being ignored. Four of the CNAs did not show up for work on Monday. Other staff members had to do their jobs. There are never enough clean towels/sheets for the patients. No towels, no showers! Money has been stolen from patients who need to have money to do their own laundry. Medications are often not given. Mom is on insulin, and today it was given to her at 10:00 when we returned from a doctor's appointment that they knew she had. When going to doctor appointments, we always ask for a history of her bp readings and blood sugar readings--they're never ready, and we're usually on a tight schedule. They seem to only be able to give you readings for that day, and they write those down on a sheet of paper. Mom will be leaving this place as soon as paperwork is done, and all I can say is keep your loved ones out of Legend Oaks.

My father has recently been there a few months and since his arrival the facility has lacked to give him proper hygiene care. There has been several times that we have asked to please shower based on their process (3 times a week) but the lower and higher staff seem to have the lack of care of experience to effectively do this small task. There has been other task where they have stated that they did shower my father but after speaking with my dad and taking pictures of his clothes it was very clear that they did not shower and forged the task. If they are being unethical about this, what are the other task they are not being forthcoming? Will write more soon!

Very disappointed with the facility ??my father is hospice with ending stage Alzimers he was put in Legend oaks for only 5 days had a very bad experience.. No one knows who [Removed] my fathers clothes, over [Removed] my father was dehydrated they don't check on them like they said they will... was with the same clothes on for 2day's....maintenance don't clean their own personal restroom I had to clean it.... they make fun of other patients I witnessed it with my own eyes.... then after my father being home I get a call from [Removed] that why did I put a bad review on Yelp because that's her reputation.. I told her it's my father who was the patient and went through hell with the lies... and she said we just tried to help you.... I don't recommend it at all ... don't fall for how nice the facility looks ...... I have been my father's caregiver for 1 yrs 24hr I know my father so good ...I even recorded my father just to have a proof of how he came back from Legend OAKS ??

It's wonderful and it's such an improvement to the place he was in before. He's doing a lot better and he seems much happier. The nursing care has been really good. I eat with him a lot and I feel the food is really good.

They were excellent and they did a wonderful job. The staff was wonderful. We had meals there and it was very good.

The whole atmosphere is nice. Everyone, from the directors down to the nursing staff, is involved with the residents. They really give personalized care. I like that the nurse’s station is next to the dining room. Everyone really cares about the residents. Anyone would be pleased with the place.
