They were great when my mom didn't need much in the way of dementia support, just couldn't live alone and needed programming and other people. Line staff 5/5, higher ups - 2.5-3 friendly to your face but profoundly not helpful. Care was good when we just needed her not to live alone or cook for herself, and she needed programs and people interaction. By the time her dementia started progressing - went down to 2/5 maybe? 1 at some point? I wish I had moved her to memory care 2 years before I did. If I had, she might still be alive - if there had been enough staff to support her so she didn't fall repeatedly - including forgetting that she couldn't get out of her wheelchair after being released from the hospital and so standing up to try and open her apartment door just to fall down - but frankly there's no way you could open those doors from a wheelchair. And if I had, her dementia might not have progressed as quickly (it also might have, no way to know) as it did if she had the support of people who actually understood sundowning and paranoid delusions and could have helped her with them more... There's a real split between line staff who are attentive and caring, and say the structure of the nursing office and the fact that you could never get anybody who understood what was going on medically on the phone. The food looked bland to me, but nobody really looked like they minded much. She did love the activities - they kept her occupied and engaged and active in the community which was really critical for her.
